Hospice of Yuma’s interdisciplinary team provides physical, emotional and spiritual care to terminally ill patients and their loved ones. The hospice team offers:, •A full-time physician Board Certified in Palliative Care Medicine who makes home visits, •Scheduled visits by experienced registered nurses for pain and symptom management, •CNA’s who provide personal care and bathing, •Social Workers who provide emotional support and community resources for patients and their families, •Spiritual support, as requested, •Trained volunteers who provide companionship and support, •Bereavement counseling and support for family members throughout the patients’ illness and for up to 13 months following the death, •24 hour on-call availability of hospice staff, •Medical equipment for comfort and symptom management, •Medications for pain and symptom management, •Acute Inpatient services at YRMC and at a local nursing home facility, •Respite care services, •A homemaker to assist with light housekeeping
